Safeguarding Transition Protocol
The Newcastle Safeguarding Transition Protocol ensures the seamless safeguarding of young people aged 17.5 and above as they transition to adulthood. It coordinates efforts between children’s and adults’ services to protect those with ongoing care needs or risks of abuse or neglect.
Children’s Social Care leads until age 18, collaborating with the Safeguarding Adults Unit for referrals, mental capacity assessments, and tailored support.
The protocol addresses complex needs, risks posed by or to young people, and emphasises advocacy, early communication, and a personalised approach to ensure safety and continuity of care.
