It advocates for a fluid, person-centred approach to safeguarding older adolescents and young adults, incorporating preventative and recovery-focused practices.
The Transition Process Guidance outlines the procedural steps for transferring cases from children’s to adult services, beginning at age 14, involving multi-agency collaboration, EHCP reviews, and joint planning to ensure seamless support for young people turning 18.
Together, these documents emphasise holistic, developmentally attuned, and coordinated safeguarding approaches to improve outcomes for young people.
