Learning and Review Pathways – Safeguarding Incidents
This practice guidance should be read by Safeguarding Partners, and all agencies involved in the Multi-Agency Safeguarding Arrangements (MASA).
The guidance is intended to support partners in understanding the options and pathways for different types of reviews:
- Part One outlines the pathway relating to Serious Child Safeguarding Incident Notifications and the initiation of Child Safeguarding Practice Rapid Review.
- Part Two outlines the pathway relating to notification of a Death of a Child in Care or Care Leaver
- Part Three outlines the pathway relating to cases that do not meet the criteria of a Serious Child Safeguarding Incident but further learning may be gained from a review
The documents below also include guidance in relation to the notification process and a glossary of abbreviations commonly used by partners.
The guidance is aimed at anyone who might be thinking about referring a case for review and those involved in undertaking or contributing to Local Child Safeguarding Practice Reviews, such as:
- Independent Lead Reviewers
- Review Team members
- Those providing information reports on behalf of their organisation
- Those responsible for quality assuring and embedding the learning from the review process.
This framework and guidance have been endorsed by the safeguarding partners and will be reviewed and updated to reflect changes in national guidance and emerging good practice.
